Conflict of Interest:
As required by Chapter 176 of the Texas Local Government Code, any person or vendor considering doing business with a local government entity must complete a disclosure questionnaire regarding the affiliation or business relationship of that person or vendor with any “local government officer” that might be considered as a conflict of interest.

Certificate of Interested Parties (Form 1295):
As required by Section 2252.908 of the Texas Local Government Code, vendors entering into a contract valued at $75K or more with a local government entity that requires approval by the governing body (GCWA Board of Directors) must complete a Certificate of Interested Parties (Form 1295).
